BACKGROUND The Centerville Community School District is a rural school district consisting of approximately 1520 students, 140 instructors, 7 administrators, 4 directors, and approximately 170 support personnel. The district is presently housed in ten (13) facility complexes: Centerville High School; Howar Junior High; Lakeview Elementary (grades 4-6); Central Elementary School; Garfield Elementary; Mystic Elementary (in Mystic, Iowa); Cincinnati Elementary (in Cincinnati, Iowa); Special Needs Education Center; Alternative High School; Centerville Commuinty Preschool; and a complex of Lincoln Elementary, the District administrative offices, and the Fleet Maintenance Facility.

DETERMINING FACTORS FOR AWARDING CONTRACT · Reliability RFP REQUIREMENTS FOR TELECOMMUNICATIONS The Centerville Community School District is requesting proposals for its digital transmission services, local, long distance and cellular telecommunications needs. All proposals are subject to the rules governing the Universal Service Fund (Erate) and the District receiving Erate approval. Vendors shall quote a best price for service and/or near and long-term leasing costs for any equipment and installation. Equipment and capacity requirements are to be the best estimates currently available. The Centerville Community School District reserves the right to modify quantity and configuration requirements if needed. The vendor agrees to sell the Centerville School District the revised quantity of items and services at the unit/per line price as stated in the RFP regardless of quantity changes. The following telecommunication services should be addressed in the vendor proposal for either part or all: · Local Service for 15 existing phone lines.
BIDDER QUALIFICAITONS The bidders must be an established telecommunications carrier.
PROCESS FOR RFP SUBMISSION The following process should be followed for responding to this RFP:

This Request For Proposal (RFP) is intended to provide a standard base from which to evaluate appropriate and cost-effective systems. The District is not obliged to purchase any system, nor to purchase the least expensive from among the Vendors. After receipt of the bid proposal and prior to signing a contract, the District has the right to modify the proposal by negotiating any changes it judges to best meet the needs of the District.

All proposals are subject to the rules governing the Universal Service Fund (Erate) and the District receiving E-Rate approval. Vendors shall quote a best price for service. The Centerville Community School District reserves the right to modify quantity and configuration requirements if needed. The following internet access services are requested: · 20 Mb/s download internet connection
